Global Service Resources is looking for an experienced Licensed Clinical Social Worker (LCSW) for a Correctional Facility in Stockton, CA.
Stockton, CA
6 months (Renewable)
8:00 AM – 4:00 PM
Payrate: $93-$95/hr
- Minimum of 1 year experience within the last 3 years
- If provider has ever worked in any correctional facility/ state prisons, please note facility and dates worked
- 2 weeks orientation/ training (Mostly Mon-Fri AM Shift)
Documents Needed:
- LCSW License
- Updated NPI number that reflects current licensure and taxonomy (If not updated, please note)
- Have valid professional liability insurance
- ID (Driver's License/ Passport) and Social Security Card
Job Responsibilities:
- This would assist in attending to those patients that are brought to our emergency department that need social work interventions.
- Interviews and assesses patients and assists throughout patient care plan
Contributing to team-based care for the management of chronic health conditions (e.g., diabetes, hypertension, heart disease, obesity, etc.)
- Assist in the detection of “at risk” patients and develop of plans to prevent further health complication.
- Document in Electronic Health Records in a timely, accurate, and clinically sound way, and communicates critical information to the medical team
Monitor health outcomes at an individual and population level.
- Identifies and provides helpful resources to patients and families.
- Participates in on-call rotation.
- Other related duties as required.

- Remains knowledgeable on current federal, state, and local laws, accreditation standards or regulatory agency requirements that apply to the assigned area of responsibility and ensures compliance with all such laws, regulations and standards.
