Job Description
Housing for Health is looking to hire Clinical Social Worker or Senior Clinical Social Worker through registry for Star Clinic.
Indefinite contract based on performance and attendance.
Hourly Rate: $46-$50/hr
Schedule: M-F 8:00 to 4:30 or 7:30-4:00
The Star Clinic is a primary care clinic in the Skid Row area of downtown that serves patients who are either currently experiencing or previously experienced homelessness and present with complex medical and mental health conditions. Clinical Social Workers are part of the behavioral health team which also consists of a clinical supervisor, substance use counselor, community health workers and medical case workers. The behavioral health team is part of the greater multidisciplinary team providing comprehensive care for Star Clinic patients. The Clinical Social Worker provides behavioral health services and complex case management to both patients who are enrolled in Enhanced Care Management, as well as, those not enrolled. Services provided include (but not limited to) completing psychosocial assessments, assisting with housing placement/retention, engagement in care, providing psychotherapy and crisis intervention.
ESSENTIAL FUNCITONS include but not limited to for Senior Clinical Social Worker:
Patient Interface 70%:
- Assesses and treats the complicated psychosocial problems of homeless and formerly homeless individuals including but not limited to their medical condition and/or functional status, untreated or under- treated mental health or substance use condition, economic instability, legal problems, and inadequate social supports, housing and transportation.
- Engages in complex case management services with patients of the Star Clinic to meet the agreed upon treatment goals.
- Participates in improving chronic disease outcomes by providing primary and secondary preventive care services and self-management tailored to the patient’s needs.
- Assists in increasing housing placement, retention, and maintaining supportive relationships with the homeless and formerly homeless individuals during care provided in clinic and at the patient’s home.
- Conducts comprehensive assessments and care planning of people experiencing homelessness and develops treatment plans that are sensitive to the patient’s diverse needs and abilities.
- Provides individual, group, and family psychotherapy and advanced level case management interventions, including but not limited to: participating in treatment groups that support health promotion such as medication assisted treatment, group visits using motivational interviewing and a trauma informed approach.
- Use of Behavioral Health Techniques including, but not limited to, motivational interviewing, cognitive behavioral therapy, relapse prevention therapy, psychotherapeutic and/or systems interventions, and trauma-informed care, delivered in an individual and/or group setting.
- Completes PHQ-2 and PHQ-9 when indicated for screening and treating depression.
- Identifies, evaluates and reports suspected dependent adult, elder, child and domestic violence/intimate partner abuse.
- Demonstrate advanced knowledge and skill in a social work treatment modality or area of practice.
Administrative 30%
- Documents written psychosocial evaluation and identified problems, treatment process and clinical outcome in electronic medical record.
- Provides consultation to the multidisciplinary team to support management of chronic medical and behavioral conditions and prevent acute care utilization through transitions of care interventions.
- Provides clinical consultation to the multidisciplinary teams on complex social work diagnosis and treatment problems.
- Participates in team huddles, case conferences, and multidisciplinary team meetings as needed.
- Participates in performance/quality improvement (PI/QI) activities and contributes to policy development as needed.
- Shares knowledge and effective practices with other Care Team members.
- Serve as acting social work supervisor during staff shortages when requested.
- Assume instructor responsibilities for graduate social work students.
