Minimum 12 months of direct patient care experience within the last 3 years (in public or private institutions).
Current and valid California LVN license from the State Board of Vocational Nurse Examiners.
Current BLS certification (American Heart Association or Red Cross with AED).
Proficient in English (spoken and written) to communicate effectively with CDCR/CCHCS.
Key Responsibilities:
Provide healthcare services in accordance with institutional policies and LVN scope of practice.
Perform basic hygiene care and administer prescribed medications and treatments.
Report patient status and vital information to RN or Primary Care Provider.
Deliver patient/youth education and document all care and physician orders accurately.
Assist with therapeutic procedures, equipment maintenance, and specimen collection.
Support RN and providers during appointments (e.g., physical exams, minor procedures).
Collaborate with mental health staff by monitoring and recording patient behavior.
Participate in patient/youth training in daily living and assist with routine treatments.
Document nursing care and patient response in health records to ensure continuity of care.
